Creating a Referral

This guide will show you step by step how to Add a new referral in the RAS module. We have even made it easier by including short videos to help.

Explanation of Sections:

Referral Source: Details of the referral.
Client Details : Clients personal details.
Contact Methods: Details of contact method for the referral.
Residential Address: Physical address details for the referral
Other Demographics : Clients other personal details (including housing).
Additional Referral information : Other referral questions.
Risk Factors : 
List of clients risk factors.
Outcome : 
Clients address details.
Program information : 
Program streams available.
Assigned Team : 
Agencies team to be assigned to referral.

  1. Locate the Add Referral option in the sub menu.
Example Add Referral.

The following fields are mandatory on this page.
* Referral Date is required.
* My Agedcare Id is required.
* Family Name is required.
* Given Names is required.
* Date of Birth is required.
* Gender is required.

  1. Enter a Referral Date.
  2. Now you can search for an existing client by selecting Search for Client.
  3. This will pop up a search for existing clients – you can search by Age Care ID, Family Name, Given Names, Alias / Preferred Name.
  4. The search results will show Name, Gender, DOB and any active cases in the RAS module.
  5. To select an existing client, click on Select for the client’s record.

Add a Contact Method

  1. Add a Contact Method by selecting +Add Contact Method button.
  2. Select Contact Type from drop down.
  3. Add Contact Details for contact type.
  4. Check (if required) Preferred Contact Method if client’s chosen contact type is the preferred method.
  5. Check (if required) Text Preferred if the client choose a text message over a phone call.
    *Note: you can add multiply contact methods by following this process.

Add Address Details

  1. Add a Residential or Postal Address for a client.
    *If the residential and postal address are the same, check the “Postal address is the same as residential address”.

Other Demographics

  1. From the drop-down list you can set the Indigenous Status, Country of Birth and Preferred language.

Additional Referral Information

  1. Enter the My Aged Care ID.
  2. Select relevant option from the drop-down list for Interpreter Required.
  3. Select a language from the Language Requirements drop-down list.
    * Note: if the language is not in the drop-down list you can specify in this field “If Other Please Specify”.
  4. Enter a comment in the Referral Comments field if required.

  1. From a drop down list you can set the Status of the outcome and its corresponding Start Date.

Assign Team

  1. Clicking on the search button, you will be presented with a Team selection pop-up window.
  2. From here you can search by Agency Name, Suburb / City or Postcode.
    Once you have located the team click Assign to select this team.

Click Save to create the referral.
