Submit A Ticket

This support request form has been provided to help lead agencies request support for their Fixus system.

If you are not a designated support officer representing a lead agency then please contact your lead agency for support.

Depending on the nature of the request we may require approval from a management level staff member before proceeding with the request.

It is extremely important that you do not put Personally Identifiable Information (PII) into support requests.

Maximum size: 1MB.
Allowed file types: .jpg, .jpeg, .png, .gif, .pdf, .txt, .csv, .xls, .xlsx, .doc, .docx.
