Service Contacts

This guide will show you step by step how to Add Service Contacts to a case in the PMHC module. We have even made it easier by including short videos to help.

Explanation of Sections:

Session: Details of a case Service Contact.
Case Session Assessment: Attach a Service Contact Assessment.
Case Session Notes: Add Service Contact Notes.

Add Case Sessions

  1. Open the Case you wish to add a service contact to.
  2. Locate the Service Contact tab in the left-hand menu and click on it to view case sessions.
  3. Now on the sessions screen click on +add service contact.
    * Session Date/Time field is required.
    * Clinician Name field is required.
    * Status field is required.
  4. Select Service Contact Type (Individual or Group)
    * Note: if Group is selected, a Group Type will need to be selected.
  5. Select a Date for the service contact.
  6. Select a Clinician Name.
  7. Select a Status.
  8. Complete all answers to each question from the drop-down list.
  9. Type in a Location of the session.
  10. Check which options apply in the Strategy section if required.
  11. Click Save to save your changes.

*With the introduction of PMHC v4, you may see some new fields. these fields should be defaulted based off the program stream you select.

Add Assessments

  1. Open the Case you wish to add a session to.
  2. Locate the Service Contact tab in the left-hand menu and click on it to view case service contact.
  3. Located the Service Contact you wish to add an assessment to. Click View.
  4. Click Assessment on the left-hand sub menu.
  5. Now on the assessments screen click on +Add Form and select from the drop-down list which assessment you would like to add.
  6. Complete all required fields (denoted in red).
  7. Complete all answers to each question from the drop down list.
  8. Click Save to save your changes.

Add Notes

  1. Open the Case you wish to add a service contact to.
  2. Locate the Service Contact tab in the left-hand menu and click on it to view case service contact.
  3. Locate the Service Contact you wish to add an assessment to. Click View.
  4. Click Notes on the left-hand sub menu.
  5. You will be presented with the Add Note pop-up window.
  6. Fill in the mandatory fields of Subject and Note at the top, as well as Category from the drop-down list at the bottom.
  7. If required, fill in any of the other fields.
  8. Click Save to save your notes.
    * Note: you can also change Visibility, priority, Category and enter a Direct Time and Indirect Time.
