This guide will show you step by step how to Add Service Contacts to a case in the PMHC module. We have even made it easier by including short videos to help.
Explanation of Sections:
Session: Details of a case Service Contact.
Case Session Assessment: Attach a Service Contact Assessment.
Case Session Notes: Add Service Contact Notes.
Add Case Sessions
- Open the Case you wish to add a service contact to.
- Locate the Service Contact tab in the left-hand menu and click on it to view case sessions.
- Now on the sessions screen click on +add service contact.
* Session Date/Time field is required.
* Clinician Name field is required.
* Status field is required. - Select Service Contact Type (Individual or Group)
* Note: if Group is selected, a Group Type will need to be selected. - Select a Date for the service contact.
- Select a Clinician Name.
- Select a Status.
- Complete all answers to each question from the drop-down list.
- Type in a Location of the session.
- Check which options apply in the Strategy section if required.
- Click Save to save your changes.
Add Assessments
- Open the Case you wish to add a session to.
- Locate the Service Contact tab in the left-hand menu and click on it to view case service contact.
- Located the Service Contact you wish to add an assessment to. Click View.
- Click Assessment on the left-hand sub menu.
- Now on the assessments screen click on +Add Form and select from the drop-down list which assessment you would like to add.
- Complete all required fields (denoted in red).
- Complete all answers to each question from the drop down list.
- Click Save to save your changes.
Add Notes
- Open the Case you wish to add a service contact to.
- Locate the Service Contact tab in the left-hand menu and click on it to view case service contact.
- Locate the Service Contact you wish to add an assessment to. Click View.
- Click Notes on the left-hand sub menu.
- You will be presented with the Add Note pop-up window.
- Fill in the mandatory fields of Subject and Note at the top, as well as Category from the drop-down list at the bottom.
- If required, fill in any of the other fields.
- Click Save to save your notes.
* Note: you can also change Visibility, priority, Category and enter a Direct Time and Indirect Time.
